WellSky and Atlantic Health Expand Partnership to Boost Post-Discharge Care

WellSky, a leading health and community care technology company, announced the expansion of its partnership with Atlantic Health System (Atlantic Health), a premier provider of healthcare in New Jersey. Building on the successful implementation of real-time post-acute performance analytics solution, CarePort Insight, in 2021, Atlantic Health is further strengthening care coordination and patient management across the healthcare continuum with CarePort Connect.

With 35 in-network skilled nursing facilities (SNFs), approximately 76,000 patients attributed to Medicare Shared Savings Programs, and nearly 520,000 patients attributed to Atlantic Health’s three accountable care organizations, ensuring positive patient outcomes post-discharge is critical to Atlantic Health’s success as it moves into value-based risk arrangements.

CarePort Insight provided Atlantic Health with real-time post-acute performance data to build and maintain its post-acute collaborative. After the successful implementation of CarePort Insight resulted in increased placements to in-network providers and improved SNF length of stay, Atlantic Health added WellSky CarePort Connect.

Now, CarePort Connect empowers Atlantic Health providers with real-time data and visibility as patients journey across the care continuum and facilitates electronic sharing of documents and communication between acute and post-acute settings. With the expansion of CarePort Connect, Atlantic Health can now bridge gaps between different electronic health records systems and gain insights beyond what is in its own EHR.

This comprehensive view is critical for effective care coordination and helps to drive operational efficiencies by centralizing access to discharge paperwork, enabling intelligent patient interventions, and facilitating Transitional Care Management follow-ups post-discharge. Additionally, CarePort Connect supports post-acute relationships by analyzing trends across care settings, identifying areas for improvement like reducing readmissions through root cause analysis, and managing patients with real-time data and care transition alerts, ultimately making it possible to enhance collaboration and improve outcomes.
